The State Patrol is responsive to the needs of Washington's citizens by providing information and access to agency manuals, reports, forms, and surveys, both on-line and hardcopy documents.

To obtain a copy of a WSP form, please use the following procedures:

1. If you wish to file a report for a collision, please complete the Motor Vehicle Collision Report (see below) and follow the instructions in the form to submit to the Washington State Patrol Collision Records Section.

• Not Empowered(578 KB) for those who wish to print the form and complete by hand.

2. If you were involved in a collision and would like to request a copy of the collision report, follow the instructions on our Collision Reports page .
3. For the Police Traffic Collision Report Overlay, see the Overlay Sheet .
   The Overlay Sheet is used to interpret the codes written on the Police Traffic Collision Report. Place the Overlay on top of the report with the numbers on the sides matched. Compare the codes to the meaning opposite the arrow for Driver 1 and Driver 2 .
4. For the Police Traffic Collision Reporting Manual see the PTCR Manual . This form is designed to assist police officers in completing reports on vehicle collisions required by the laws of Washington.  Questions concerning this manual should be directed to the Collision Records Section at (360) 570-2355.
5. Law Enforcement: To order any of the Police Traffic Collision Reporting forms, please complete the PTCR Order form in Word or PDF format .
6. For all other collision reporting forms, please contact the WSP Collision Records Section at (360) 570-2355.
7. For the Uniform Washington State Authorization to Tow/Impound and Inventory Record, please order through the State Printer's Web site at http://www.prt.wa.gov/ . Once there, click on "Online Ordering," then "General Store."
8. For questions about criminal history background checks, related forms, and Washington Access To Criminal History (WATCH) on-line, contact watch.help@wsp.wa.gov .
